f.  Component/Parts in Stock at All Levels (Depot and Others) Including War Reserves . Upon receipt
of this TB, the material condition tags of all items in all condition codes listed in paragraph 6 shall be annotated
to read UH6099ASAM08 Air Industries Bellcrank Support Assembly not complied with.
(1) Wholesale Stock. Upon receipt of this message all serviceable items (condition codes "A",
"B", "C", "D", and "E"), identified through the paragraph 8 inspection shall be placed in condition code "J" and
tagged with a suspended tag/labelmateriel, DD Form 1575/DD Form 15751. Do not remove original condi-
tion tags. Only those paragraph 6 components that have more than 500 hours useful life remaining, based
on the life specified in paragraph 9, will be issued. Report compliance with this TB in accordance with para-
graph 14.d.(1).
(2) Retail Stock. Upon receipt of this TB Commanders and others maintaining retail stock at
installation level and below shall contact the supported Aviation Unit to perform the inspection required by
paragraph 8 and the correction procedures of paragraph 9. Only those paragraph 6 parts that are known to
have more than 500 useable hours remaining, based on the life specified in paragraph 9, may be installed
on aircraft. Disposition of materiel will be IAW paragraph 10. Report compliance with this TB IAW paragraph
14.d.(2).

Task/Inspection Suspense Date. Within next 10 flight hours/14 days.

Reporting Compliance Suspense Date. No later than 30 June 1999 per paragraph 14.a of this TB.

Summary of the Problem.
a. The Bellcrank Support Assembly, being delivered under production and spares contracts through
Sikorsky Aircraft Company, was manufactured by Air Industries, an untested source. A reduced life of 5,000
hours must be set on Bellcrank Support Assemblies, P/N 7040008162042/045, and a reduced life of 2,200
hours on Bellcrank Support Assembly P/N 7040008162043, from Air Industries, CAGE 8L513. These re-
duced lives will hold until the required engineering tests are complete. The subject part is also known as the
main support bridge and/or bridge support.

This TB has three purposes.
(1) Require a one time inspection to locate Bellcrank Support Assemblies, P/N
7040008162042/043/045, manufactured by Air Industries, CAGE 8L513 which are installed on aircraft or
in supply.
(2) Establish an interim retirement life of 5,000 hours for Bellcrank Support Assembly, P/N
7040008162042/045, manufactured by Air Industries, CAGE 8L513.
(3) Establish an interim retirement life of 2,200 hours for Bellcrank Support Assembly, P/N
7040008162043, manufactured by Air Industries, CAGE 8L513.
