4. Summary of Problem.

a. The Main Rotor Shaft Extensions manufactured by the Purdy Corporation (Cage Code 15152) and

Fenn Manufacturing Company (Cage Code 82001) was restricted to 2100 hours of service by Safety

Action Message UH-60-97-ASAM-09. Required testing has been completed and the parts are equivalent

to the original manufacturers components. The service life of the subject components is revised to the

original 14,000 hours.

c. The purpose of this TB is to eliminate the restricted service life of the Fenn Manufacturing and

Purdy Corporation Main Rotor Shaft Extension Assemblies, P/N 70351-08186-043, implemented by UH-

60-97-ASAM-09.

8. Inspection Procedures. Inspect time change DA FORM 2408-16, DD FORM 1575-1. If form

shows 2100 hours for specified part due to the previous ASAM, go to paragraph 9 correction procedure.

If time change record does not show 2100 hour life for specified part then inspection is complete. If DD

FORM 1575 or 1575-1 is attached to wholesale or retail stock item, go to paragraph 9 correction

procedure.

9. Correction Procedures. Annotate the aircraft time change DA FORM 2408-16 to reflect the

original service life of 14,000 hours for the specified M/R Shaft Extension Assemblies.
